Medical firms, different connections or hubs. They are very expensive and rare. The very vast majority of doctors will completely refuse to do surgery on you, unless if your head is missing half of it's skull.

He always posts photos of his patients after surgery. It looks legit. And this dude isn't niche local surgeon. He's one of the best eye surgeons I've found
