Loading the upper epiphysis of the femur is virtually impossible since it locatedwithin the hip area. So we must be content with loading only the lower epiphysis ofthe femur. Through e7perimentation we have found the easiest way to locate thelower epiphysis of the femur is to pinpoint its location in relation to the "nee cap.



The "nee cap is a special bone in the human body in that it is sesamoid bone$meaning it is a bone embedded within a tendon. The area around the "nee &oint is acomple7 area containing the epiphysis of the femur as well as the tibia$ and manytendons and ligaments as well. This is why it is especially important when loading tobe responsive to any pain e7periencedB if you e7perience any pain then you aremost li"ely pinching a tendon and should immediately release pressure and tryagain. To identify the femur0s epiphysis simply pinch your "nee cap with the thumb andinde7 2nger. Cinching and identifying your "nee cap should be relatively easy. =romthe diagram shown below you can see the "nee cap rests directly upon theepiphysis of the femur$ so to identify the correct femur loading position$ simply slideyour thumb and inde7 2nger o! of the "nee cap$ and now with your thumb andmiddle 2nger (since it0s a bit of a stretch with your inde7) you should feel a boneprotruding from each side of your grip. This is the femur loading position.


Tibia "piphyses
:. The Dnee The tibia0s upper epiphysis is J-ST below the "nee cap. The easiest way to 2nd itsepiphysis is to cusp the shin from behind with your hand and slowly "ic" your lowerleg (shin) up and down. %ith your other hand feel for the protrusions on each side of the leg that are moving with the motion of the "ic". If the bone feels li"e it is notmoving then it is most li"ely the femur. %e must stress that it is a 2ne line betweenthe two bones ends$ but with practice you should be able to become pro2cient inidentifying which is which.


8. The n"le=or the an"le the most important area to focus pressure is on the inner an"le (onyour big toe0s side). This is the epiphysis of the tibia$ while the outer an"le boneprotrusion is the epiphysis of the smaller 2bula. The 2bula does not contribute asmuch to overall height as the tibia does and thus we want most of the pressurefocused on the inner an"le.%hen loading the an"le$ it is sometimes painful with direct metal to bone contact ifyou are using a c+clamp. Through e7perimentation we have found the most e!ectiveloading position is one where the clamp is slightly staggered so that the screw is notdirectly on the outer an"le but &ust in front of it. Some users have also opted toutili>e a barrier such as a paper notepad or eraser to further reduce pain on theouter an"le.
