I mean will a lower BMI and some lean muscle be helpful in approving sex appeal and might my face look less childsh? I'm pretty over weight and i wonder if that is partly the issue. 220lb at 6ft.

surgery’s: lower third is disproportionately smaller than your upper third. You have a rather short skull. Not a short face syndrom but you could benefit from genio or chin wing or whatever to add chin height. Would also improve your hyoid bone so you look more forward grown.
