Im being asked to declare my package from Indiamart

I recently ordered dut and oral min from Indiamart. I do not have a prescription. “Please provide a goods order document. The document must include the product name, quantity, value, total shipment value, shipping costs, and the recipient’s details.” Tf should I do. Im located in Lithuania.
