Hey everyone! I’m getting bimaxillary (double jaw) surgery soon and wanted to share my surgical blueprint to get some feedback from the community before everything is 100% locked in. I’m uploading my scans and pictures so you can see my baseline.


To give you some context on my starting point: I have a really narrow jaw. Profile-wise, it's not a typical recessed jaw—my oral maxillofacial surgeon actually pointed out that my chin itself is already very strong and well-developed, but the overall framework is extremely narrow, asymmetric, and sitting in a Class III relationship.


The Plan (Set in Stone)


We are going with a Surgery-First protocol (surgical hooks put on right before, with post-op braces used for touch-up detailing). Here are the exact 3D VSP movements already set:


Maxilla (LeFort I): +7.4 mm anterior advancement, a 4.0° clockwise (CW) pitch rotation, and leveling a ~4.0 mm occlusal cant (lifting the right side/lowering left) to level out my smile.


Mandible (BSSO): 6.0 mm yaw shift to the right to fix my midline asymmetry. Because of the clockwise rotation and bite correction, this results in a net +2.1 mm forward advancement at the chin point.


My Main Dilemma: Should I add a Sliding Genioplasty?


This is the last tweak I'm debating. My surgeon mentioned that because my chin is naturally quite strong, we don't really need to advance the chin with a genioplasty.


However, my concern is about relative balance once everything moves:


Since my upper jaw/midface is moving forward by 7.4 mm, but my lower jaw point is only advancing 2.1 mm forward, is my currently strong chin going to look too weak or flat relative to the new, advanced upper jaw?


My thought process is that a chin that looks "strong" on a recessed/narrow baseline face might suddenly lose its relative prominence once the midface is brought forward over 7 mm.

I actually did palate expanders back in 2022 along with Invisalign to try to avoid surgery, but the underlying skeletal issue was just too significant. So my orthodontist, who like handled that came together with my surgeon and built this plan.

Why don’t u try get segmental lefort
