Usually it’s pointless. Most people who need MSE are Lefort 1 candidates.

Wouldn't invisalign come after mse in that case?

I'm currently using invisalign btw, shit is annoying I wish I just got braces, I only wear the aligners at night

Yea after. I dunno if I’m a candiadate yet I’m gonna ask but I think I have narrow palate. I just want smile gains tbh

I don’t know why users are against MSE. Functional gain, palate gain, and subtle but natural-looking gain in multiple features like cheek bones and mouth. If I was getting it I would ask to get as much expansion as you can so you get the most benefit out of it.

I already have wide zygos tho so I’m scared of over expanding. I will be conservative

Usually it’s pointless. Most people who need MSE are Lefort 1 candidates.
MSE expands the palate and nasal airway much more than lefort one unless the lefort is a 3 way segmented. Even then a 3 way lefort wont produce the expansion up the length of the nasal volume the way an MSE will. The optimal procedure in many cases is to get MSE, then follow up with MMA.

MSE is spectacular for that, though many orthos will want a health/breathing/occlusion reason for providing MSE as aesthetic reasons are subjective and patients can be uncomfortable with changes to their appearance, even if the changes are objectively an improvement to outsiders.

How do they deal with asymmetry?
What if the palate doesn't split?

Gotta take them out every time you eat. Every time you hve to switch aligners it’s a fucking struggle. Lastly, it gives me a lisp. Braces mog

Maxillomandibular Advancement - double jaw surgery. MSE expands laterally, the MMA expands forward.

Usually they can make the lower teeth tilt to match the expansion of the upper jaw. There are one or two rothos who expand the lower jaw along with the upper when doing MSE but its rare. Most people dont need it.

Not unless you already need it. Look at your profile, is your upper lip further forward than your forehead? In that case and no sleep breathing issues then probably not.

My suggestion is to have conservative expansion. I got 8 mm when the ortho said 6mm and now I think it was too much. I discovered that my feeling of being boneless was partly due to have not payed attention to my facial bloat. Dehydrate for a few days before considering if you want wide expansion. It might affect your ability for hollow cheeks.

In my experience MSE only has a tiny net positive, there are changes but they were more of a trade-off. Your midface might become flatter due to more surface area for skin. Id have done 5-6mm if I were to do it again since I STILL have a cross bite dealing with invisalign.

Also consider if you have maxillary cant. If one side droops more than the other, the drooping side will drop even more.


Also facepulling is a meme if you are not flat faced.

There are molar bands that have attachment sides, you use certain rubber bands that stretch but also have resistance. You'll end up wearing an autistic appliance like a facebow or neckbrace.

Not much you can do except osteotomies for maxillary cant.

These kind of aesthetic concerns are really dependent on what your are starting with. It's too complex to get into without visual reference. Patients expanding more than the ortho recommends and then regretting it is quite a common story. MSE has a tendency to accentuate any existing symmetry, such as a cant. Again it all comes down to your face.

Get a good ortho with MSE experience, ask him how many cases he's had in total minimum 250 is safe
