smash or pass really is that simple
but if more detail it mostly comes down to health indicators and subjectivity

white teeth, good hair density, hairline,good bone structure/development, skin production etc etc

What are the most important factors in determining whether someone is attractive?

it is too complicated to list it so easy, i would want to generalize by saying that being attractive is primarily having a face with good ratios, but even with the ideal ratios you still can look like shit because of soft tissue problems like fat and bloat which can kill all the attractiveness, so the question can not be answered like you expect
