@RealSurgerymax did you ever have a case where infraorbital malar implants dragged down lower eyelids (maybe tightened due to a previous canthoplasty/pexy) instead of lifting them up?

No although it is possible if you don’t follow some principles.
Never happened to one of my cases. Although eventually it might

What are your thoughts on getting a second wedge genio for vertical height if you already have gotten one? I know this will make the step off worse but can’t you get fat grafting or filler afterwards to fix that?

Need to see scans and pics. It might not make step off worse and you can bone graft. There are many planning strategies to balance result and risk

Whats your opinion on facegenics? And their upcoming protraction?do you think it can do more than 3-4mm protraction?

if i need a referral from a doctor to get a ct scan what do i tell them? i doubt they will give me one if i say i want jaw implants

Why though? Can’t the implant be designed to add vertical height? Eppley does it all the time.
