He seems to care a lot about jawlines and cheekbones, although I think all these transformations are done with fillers. Is there a way to achieve these great results with implants or other permanent procedures that don't fade away after a year or so like fillers?

The advantage of filler is its ability to create angularity. It sits directly under the skin, not under layers of muscle and other potentially thick soft tissues. The downside is clear: costly over time and not permanent, with the lingering concern of long-term filler migration.

The advantage of implants are permanent, larger volume changes, and with custom design precise dimensions - but they are less predictable in final outcome because they must be secured to the bone and sit under many soft tissue layers that can blunt significant changes to angularity.

The possible solution are custom designed soft tissue implants that are placed under the skin. These can create angularity in a way more rigid implants that are fixed to the bone can't. Possible disadvantage is managing to stabilize such an implant. In my opinion, these custom soft tissue implants may be the future.

Igor Alves is just a dentist who got famous by applying fillers. He don't make any other procedures other than what he call "facial harmonization". But he injects crazy amounts of fillers to create jaw and cheekbones definition, obviously over time his patients will look bloated
