As most people know, AIs can have significant side effects and should be avoided when possible. So why is 500 mg test-only considered the standard first cycle? Why not add a DHT derivative like Primo to help manage estrogen instead? Wouldn’t 500 test + Primo make more sense than relying on an AI for a first cycle ?

Because you want to find out how you react to one compound at a time. Test+primo is a fine 2nd cycle.
aromatization of E2 is highly individual, and figure out how you respond to a moderate dose of test is good knowledge for future cycles.

Lmfao, estrogen is vital even for dick function, also collagen and elastin synthesis including joints, joint lubrication, SCALP HAIR GROWTH, maintenance of subcutaneous fat deposits and skin generally same with tendons and ligaments(elastin and collagen), main hormone that maintains bone strength and density as well, T mainly works for bones in men via aromatization to E in bone tissues actually. Extremely important hormone that you want to avoid suppressing as much as possible.

there absolutely is nothing for lipids, blood pressure, e2, or mitigating your stupid high dose. 200mg alone would be a better first dose to do with no ancillaries. You've also not taken anything to assist with test uptake and usage so your effects will be reduced to notjinf smart about that cycle

Some doctors literally put some old guys on TRT on AIs "just because", like what the fuck? If estrogen wasn't a problem when natural T levels were high when younger why would it be a problem for an older guy? Weird shit as fuck.

because it needs to stay proportional, high test has its upsides and downsides same as high e2. We take drugs to mitigate the bad sides of high test because we like the good sides. For E2, someone figured out that instead of taking shit to mitigate the bad side of high e2, it's easier to just lower it to regular levels with an ai.

Ye but on TRT estrogen shouldn't be an issue if you're trying to restore the levels of your youth circa, when estrogen wasn't an issue either. Just more liable to cause side-effects IMO. Better check estrogen levels and symptoms before deciding imo lol. Estrogen is a very good hormone for men when not in excess. Exists for a reason.

You're missing the entire point. Many people can run 500mg just fine without any AI at all if they're low aromatizers.

200mg is a replacement done for many. Have fun shutting yourself down for a replacement dose.
