Seen this from Eppley, and was wondering if this is worth it instead of a small implant? Does it last forever / not migrate? The article is about brow augmentation but it seems it has application anywhere in the face

I had HA paste applied to my cheeks during bimax. My surgeon said it does not migrate, it’s not filler, meaning it’s not just floating in there (at least the way he does it, he doesn’t inject it), it’s applied to the bone and becomes bone over 6-8 weeks. It lasts a lifetime and does not need further application. Again not sure about brow bone I just had cheek augmentation with it.

Oh yeah. I’m still quite swollen tho. It’s only been like 2.5 months. But my cheekbones are popping through the swelling and I can feel them when I wash my face, they are way more projected than before. Happy so far!

@Whirr22222 - there's a result on Gunson's instagram of someone who did HA paste to their cheeks. The outcome was underwhelming.

I once believed that hydroxyapatite paste/granules had the potential to be a superior alternative to facial implants, specifically on the cheekbone area. However, after consulting with several surgeons, I learned the following information:

- Accurate placement of HA on the cheekbones is challenging. Partly due to the difficulty of access and partly due to the material itself
- There is a risk that HA can settle unevenly, appearing lumpy. This is a problem both aesthetically and also in terms of your health, as it could lead to irritation of the soft tissue (redness, swelling or even infection)
- Surgeons often apply HA with a lack of precision, essentially "slapping it on and hoping for the best." Thus, it is not suitable for detailed shaping.
- It's also not ideal for large alterations. It's not like with filler where you can get away with putting a ton of it on the destination site.
- Removing HA is difficult, as it integrates rapidly and almost permanently into the bone. Surgeons may struggle to completely remove it, often having to scrape it off.
- Bear in mind there are different HA pastes out there with new ones having better qualities than the older ones
- What it is decent for, is as a second-best substitute when autologous bone isn't available and you want to bridge an osteotomy step-off or notch. It's also a tiny bit better on the jaw compared to the cheekbones, as access there is easier

I think this can be true, but you’re also taking a singular Instagram case and applying it as fact across all patients…Gunson has done legitimately thousands of HA cheek augmentations, I literally asked him how many, he’s been doing it for decades and has published research on efficacy based on their thousands of cases, he handed me the research paper in the office lol.

Just like any surgeon and any surgery not every case is a home run. There are complications. But most of his cases are a win and the HA looks better than what you had before for no issues short or long term.

Tbh i was only gonna go for very small amounts anyway. E.g a few mm on gonion, few mm on cheekbones, few on brow. I feel like that cant really go wrong

I know all about Gunson.

I've actually never seen a good HA result on the cheekbones btw and I've seen pretty much every one of them online, in research papers and with in-person consultations. It tends to give a feminizing appearance to the cheeks... because the surgeon isn't normally able to place them high enough on the cheekbone and not with the angularity you need

Hmm. Might just be preference then. He showed me a ton of examples and they all looked masculine and attractive to me. Without the paste, considering the huge advancement he does for most men, they’d have looked like a dog. I don’t disagree with you that shaping in the OR seems a bit sketchy but I can’t complain with my results so far. Time will tell further I guess but its light years ahead of my cheeks pre op.
