My canthal tilt is kinda neutral - positive. But in my childhood pics it's noticeably positive. I don't understand how. Is it possible to lose your canthal tilt?

yeah canthus muscles weaken over time but I don’t see how someone could have stronger canthus muscles in their childhood jfl
in all seriousness tho it’s prolly hooding and/or fat pads in the region around your eye area that’s contributing to a more positive perceived canthal tilt

So the actual tilt didn't changes but maybe the hooding ?

most likely, I don’t think someone can have weaker muscles in their childhood as compared to the present
unless you spend 8 hours a day in the sun and/or rub your eyes vigorously daily it’s not possible for your orbicularis oculi to weaken
rather it should be the opposite
