In almost all cases if you have a negative orbital vector it's usually an indicator of recession in the infraorbital region of the maxilla, you will likely need both implants + fat grafting (to account for the addition of mass) to resolve this

Because if you don't have more fat to balance out the new infraorbital mass, you will be at risk for looking uncanny and having implant show.
