Do you also design the bimax itself?
Not with the Italians. They can handle that themselves. I do the bimax planning with Turkish surgeons and all plastic surgeons (not maxfac) that I work with.

Would you recommend the Italians over the Turkish or are they about the same?

I only work with surgeons I trust fully. You are welcome to consult them individually and make a decision yourself but dont bring them up to each other asking for second opinions etc once you are under one's care. They are independent and don't work together.
