Is there any case of someone who has actually died/gone blind because of OBO

People here say that it’s an extremely risky procedure but I would like to know if someone actually did die/go blind because of it

Yes people have died from the standard transcranial version with the brain exposed.
However in the Giant Version its modified without brain involvement using a patent-pending guide. No one has died from that version.
Blindness no.
