Isotretinoin cumulative dose

We know in medical practice docs look to aim for a cumaltive dose with some formula. This is apparently supposed to be when accutane can no longer shrink the oil glands further. However I see people running it for 2-3 years. Is there any reason to take a large cumaltive dose over several years? Or is this more so just for the temporary oil suppression while on it (almost no acne while on it)

I’ve take accutane from a dermatologist twice and the cumulative dose part is pretty dumb at least for me. My acne came back a few months after I stopped (albeit not as strong). The reason some people microdose over a long period is simply to avoid any acne at all for said long time period, while having the reduced side effects from the low dose.
