What are the advantages and drawbacks of this type of Le Fort 1 cut, sliding the upper jaw forwards and upwards, compared to a straight advancement? Under what circumstances can diagonal advancement not be done? What surgeons perform it?

This diagonal slide appears to be a solution for those who’d like to shorten a long or downward-grown face. See the lines drawn over the skulls to show how the mandible swings upwards, resulting in shorter vertical facial height.

Thoughts? Is this the long midface solution we need?

i.e. you're not actually shortening the midface, just the lf1 area which will at best give you an old man smile

Not necessarily, you can have a net CCW rotation where there is more posterior down-grafting than anterior downgrafting.

But I agree that you’re not fundamentally shorting the entire midface in your example, just the philtrum.

CCW rotation =/= upwards. I’ve seen LF1 and bimax cases in which the CCW rotation is done by moving the back of the maxilla downwards while the front of the maxilla stays at the same vertical level.

Yes, anterior can remain unchanged.

How do you persuade surgeons to impact your maxilla upwards? I’m kinda obsessed with compact midfaces so I need one. If a surgeon refuses to impact and suggests a fucking downgraft instead, I’ll be pissed.

Posterior and Anterior impaction

Posterior impaction = CW
Anterior impaction = CCW

Anterior impaction is really only reserved for cases with excessive gum show so I’m not sure on this one.

I don’t think it’s possible in the way you want but I’m happy to be corrected by others if wrong.

What if I get a lip lift to induce excessive gum show? How do you persuade a surgeon to give you a lip lift if they know it’d cause a gummy smile?
