Because it's invasive as shit and has a decent chance of being fatal for the patient, not necessarily something you'd want to happen to your client as a surgeon. From what I've heard bimax (lefort 1 + BSSO) is brutal recovery wise, let alone a fucking lefort 3 which involves cutting your entire face open.
