His hair shape joining his beard is making his face round. If he’s going to do surgery surely he would

1. get a fade that blends into his beard, making his head shape squarer

2. use a retinoid. In fact a retinoid + a whole skin routine, completely remove the scarring/wrinkling on his forehead

3. lose weight, his face shows signs of puffiness and over weight.

4.trim his beard down and keep it clean

5. pluck the middle of the brows

So many soft maxes he should be doing in the meantime while he waits for surgery
