Your lower eyelid doesn’t seem to be properly covering the sclera at rest, which looks more like actual lower lid retraction rather than just volume loss alone. Some type of lower lid support/retraction repair would probably make more sense than only doing fat grafting.

Fat grafting can still help with hollowing and support the lid-cheek area a bit, but by itself it usually won’t fully correct a lower lid that sits too low.

Do you think LER should be my first surgery or do i have other major falios that need attention?

I actually think the brow ridge is the first thing to fix, since it looks relatively under-projected and gives ypou high set eyebrows.

Lower eyelid support or retraction correction would still be something to consider for the scleral show, but in terms of overall facial fixes, brow implants would likely come first before the lower lid area.

From the first pic I couldn’t really see the brow area clearly, so I think I misjudged it.

From the new pic, it doesn’t look like the brow bone is the main issue. The bigger thing still seems to be the lower eyelid/scleral show.

The brows just look a bit thin, not really a structure problem. So something like minoxidil could help with making them thicker, while the main thing to focus on would still be the lower eyelid area.

Thx for the help man. Iam also 15 so i hope puberty might help but surgery is probably inevitable.
