Lower third/mouth/chin advice?

Want to improve my lower third and mouth area. Plan on getting a bimax + genio, but also really dislike the paranasal area. Mouth/lips/nasolabial fold can look really bad depending on smile/cheek position, especially on my right profile. I'm not sure how a bimax would effect the mouth/lips exactly, but I think a wider mouth would be desired as well. Considering looking into SARPE then as well. Attached different angles of face, CT scan, as well as potential bimax movements from maxiofacial consult.

Your chin is recessed, but it's not just the chin, it's your whole lower face, which looks like it is slightly sloped backwards. The deep mentolabial fold is evidence of this.

This is speculative, but from what I have seen, a lot of guys with fat/fleshy faces (like you) seem to benefit from maxillary downgraft. So your ideal procedure would probably be a bimax procedure which both gives you more vertical height to your mid/lower face, as well as a CCW rotation that will harmonize your lower 3rd. So bimax with maxillary downgraft and CCW rotation. I don't know if you could get both of those features in a single jaw surgery, though.

Will look into maxillary downgraft, thanks for that. Bimax/CCW rotation consultation ceph images in the post, any thoughts on those movements? Also any thoughts for genioplasty movements? I know there aren't pics, but also feels like my upper maxilla lacks projection too.

If you can't get a maxillary downgraft, bimax+CCW would still benefit you a lot. You could also add vertical height with a genioplasty.
