Hi,

I'm M37, 178cm (5'10) height, 81kg (179 pounds) body weight. I would like to improve my facial appearance, so my face looks on first place more younger, healthy and vibrant, and if possible, attractive.

What would you recommend me to do?

thank you for any opinions and recommendations.

Below is the dropbox link to pics and xray. I would kindly ask, not to paste pics here.

thanks guys,

Lose weight first then get surgery. For u i would recommend a deeplane face and neck lift, lip lift, fatgrafting to the under eyes and nasolabial folds, bimax with CCW rotation cuz ur jaw is a bit downward grown, lateral canthopexy to fix ur droopy eyes and increase ur pfl and a eyebrow transplant for denser eyebrows

It is cliche advice and not a surgery but you are very obviously high body fat, a good start would be to get lean

once your face is lean from there you can make a decision on what actions are neccecary, would let you identify your falios and address them accordingly, such as fat grafts face lifts fillers etc, youre way too old to be a candidate for actual invasive/expensive surgeries like jaw movements implants etc the ROI would be too low and recovery too long

People get invasive cosmetic surgeries even later in life, but yea
You need to focus on soft tissues, you look too old for your age because of the bloat and sagging, you will need a face lift and skin rejuvenation

So to address old look, I would need to focus on soft tissues and not on bones? Or, bimax+CCW wouldn't significantly improve old look? I thought that my old look is due to possibly recessed bones.

If you have money and time to work with bones, then do it. But even with that, you will need to lose weight and do some procedures for skin tightening

I'm working on losing body fat. It's a very slow process and it could take me months to get those 5kg/11pounds down.

Now first, maybe stupid question - you're recommending me to lose body fat, which is an obvious step, but at the same time you're also recommending me fat grafting? How does this go together?

Otherwise, since I'm apparently too old for bimax+CCW and implants, I can forget creating impression of forward grown face and ogee curve.

I just explained what each surgery would do. The soft tissue surgery’s will fix the old look and the bimax will just fix ur downward grown jaw

fat grafting was a example of a likely proceedure you would need, you would need to be lean first before identifying if/where you need it performed

for example if you still have those very obvious nasolabial folds after leaning out then fat grafting would be a good option for fixing it

trimax

Bimax with as much forward movement and rotation as aesthetically possible. This will stretch out your soft tissue and provide more bone support. It won't work miracles but it will improve things quite a bit. If you're lucky it can also improve the submental area and neckline a lot. Also looks natural, unlike most facelifts

Obviously you need to lower bodyfat as well

For loosing Bodyfat hopp on reta or tirze. And definitely on ghk cu it will give u dome volume back. Then you can look for surgeries. Its never too late for blackpil.

You are clearly recessed, you need double jaw surgery+(possible) genio. There's no point in touching soft tissue before you know how it slopes over your new bone structure after the trimax. You're not "too old" for a trimax at 37. Christ.

hes 40 years old, unless he has actual airway issues theres just no way to justify getting a major extremely invasive trimax intended for deformed people cant just throw out trimax for everything

I agree that he is clearly recessed but u gotta be realistic at the very most he should be getting is a genio or implants not a lefort and bsso with large movements is just a comical reccomendation for a 40 year old

I've had few consultations with maxfacs and plastic surgeons, where my main concern was old and non-vibrant look. The general consensus is that bimax wouldn't address much of the problem, except partly easing out nasolabial folds. From what I understand, I have vertically descended fat pads in the midface area and a lack of volume in the same place. In order to address these, I'd need to have some kind of facelift, which is a domain of plastic surgery.

Here is what surgeons said:
  • maxfac surgeon 1: too high morbidity for bimax, too young for facelift as it would create "pulled" look. Cheek implant was recommended.
  • maxfac surgeon 2: facelift. Visit plastic surgeon.
  • maxfac surgeon 3: facelift. Visit plastic surgeon.
  • maxfac surgeon 4: bimax+ccw+cheek implants.
  • plastic surgeon 1: deep plane facelift + lower cheek fat pat removal + canthopexy
  • plastic surgeon 2: facelift

Besides all the osteotomies and lifting options that have already been brought up, I would strongly suggest to look into diluted Radiesse, Sculptra, Tazarotene/Tretinoin, Calecim products, Microneedling (Derminator 2), chemical peals, CO2 laser treatment ... those are the things that will give you a youthful, healthy glow. Of course, they will only come as a complement or finishing touch to the aforementioned surgeries. Without the latter, they would almost be like a band-aid on a wooden leg.

You have to understand that most normie surgeons are retarded.
You are clearly recessed. You need bimax.

Get your skeletal foundation sorted first, then start messing with the soft tissue.

Implants on a recessed face is just absolutely mongoloid. That bimax surgeon should be executed.

I've asked chatGPT to make a morph of bimax+CCW, including some midface soft tissue work. Do you think this is achievable:

SURGERY MORPH

?

Something like it, sure. not bimax alone. You'd need some kind of facelift after the bimax+genio. Is money an issue for u?

I made an in-person appointment with maxfac. What should I ask him, beside flooding him with chatgpt morphs?

U should most definitely not flood him with morphs, maxfacs dont like that

Maybe. There must be a reason why most of maxfacs I spoke with, suggested me facelift. Of course there are exceptions, like some maxfac, who suggested me lf3. Made me lol.

This is interesting:

Reverse facelift: the most durable and natural facial rejuvenation method

A large proportion of cases of facelift are made following an erroneous diagnosis: patients go to the surgeon looking for a solution for the sagging of their soft tissues, and the surgeons usually advise a facelift, when in fact the lack of tissue support in many cases is due to a lack of projection of the patient's bone structure.

The facelift is a treatment indicated to rejuvenate the face in people who have a well-balanced bone structure and have soft tissue sagging. On the other hand, when said sagging is due to a bad placement of the facial bones, the facelift achieves unnatural result and/or it ends up accentuating the lack of harmony of the patient's face.

By projecting the facial bones to their ideal position, the maxillofacial surgeon manages to provide a correct support to the soft tissues, giving tension to the face in a very natural way, while achieving a correct balance of the patient's features.
When the patient does not have a good bone projection on the facial bone structure, orthofacial surgery with bimaxillary advancement, or reverse facelift, is the indicated treatment. In this surgical procedure, the surgeon advances (and if necessary, rotates) the mandibular and maxillary bones in a block. In this way, by projecting the facial bones to their ideal position, the maxillofacial surgeon manages to provide a correct support to the soft tissues, tensing the face in a very natural way, while achieving a correct balance of the facial features.

Thus, a reverse facelift achieves a great rejuvenating effect of the face and a greater definition in the line of the neck, the mandibular contour, the chin and the lips.

He needs both. He is recessed, clearly. This best course is to fix his skeletal base before altering soft tissue

Which one?
Some local surgeon, not a big name internationally. However, I've already online consulted with Raffaini, Ramieri, Pagnoni and Zavattero.

First of all u need to lose bf
After that if ur into surgery i suggest a deep plane face lift abd neck lift also go for an aqualyx or lemon bottle seesion that's for lower third
Ur midface looks flat i suggest a Composite Midface Lift or a micro fat graft
U need a temporal lift and a Botulinum toxin session across ur corrugator supercilii and frontalis muscles that should hide ur forehead lines
That's it i see uee u could go for full orbital implants and zygo impalnts but these are optional
U could get these surgeries as a package in turkey
