My speculation: He did serious PEDs for years to retain a ripped body that does not come naturally to him, that fucked him down in some way, most likely Liver damage.

He looks 45 there, and only that young because he has no grey hairs. He is very likely dying his hair, and would look 52 there with natural hair.
