He could be a bum, beggar, homeless person, etc. in Romania for example they don't receive everyone, because the bums try to exploit the system, they call the ambulance claiming to be sick and just use it as a taxi, or they try to get free drugs, sleep inside the hospital during cold nights, steal stuff, etc... while the tax payers suffer because of these parasites.
