Mandible pemolar extraction for trimax

I am getting trimax in roughly 6 months from now, my movements will be around 8-10mm bsso, 4-6mm lf1, and 3mm genio with downgraft for my short chin, also around 3 degrees of ccw. My ortho is recommending premolar extraction, and Ive read and see how that extraction recesses you even more but if I am getting trimax does the extraction make sense? From the research ive done so far it seems like for movements more than 6mm on a bsso requires premolar extraction for best bite post op.
 
Class 2 skeletal class, but I have retrusion in both mandible and maxilla, just more on my mandible.
 
how are you getting it covered by your insurance? i’ve had my bsso denied twice already.

I'm still in the process of getting it approved, but I convinced my primary care doctor to give me a referral to a tmj surgeon as I have tmj issues, and I made sure that this surgeon in network was also a maxillofacial surgeon. And when I showed up to my consult I told him how I was here for bimax so me submitted a surgery authorization about a week ago, I should hear back soon.

didn’t know you have tmj issues, i honestly wish i had some kind of lower jaw problem so i could qualify, i had to cope with invisalign & chin filler instead.

Yeah ive had tmj issues for about 7 years now, and have had years of different types of treatments so it made that referral quite easy. Hoping for the same results for the bimax auth. Complain to your doctor about sleep issues, get a referral for an at home sleep study and have someone you know with apnea use the machine. Or for a more guaranteed longterm method, change insurances to an expensive maxed out plan, referrals are a lot easier and far more likely to get approved and you could get the surgery dirt cheap since insurance covers majority and its hassle free.
 
it’s too late unfortunately, my ortho already asked me if i have sleep apnea & like a idiot i said no, he said we wont authorize a sleep study & i can’t even change insurance plans since i’m a dependent. my best bet is to finish my invisalign & go to turkey next year & pay $10,000-15,000. im mirin ur dedication though, hopefully the ascension goes well.

You can always bullshit being oblivious and act like you didn't know what sleep apnea was. Talk to your ortho or pcp and say that you thought it was normal to wake up a 4+ times a night. There is always hope. Im not sure where your from but im in the states, and I would never consider a surgery from out of state. WAY too risky, id rather pay premium with a doctor in states than go to a high promising foreign doctor. You look at these LL clinics for example, selling false hope of way higher than recoverable height returns from lengthening and dirt cheap prices compared to the states and permanently crippling people. Always better to be safe than sorry.

yeah maybe. i have my next invisalign appointment on march 24th, i’ll try to jester my way into getting a sleep apnea test. im from the states as well & i’m also a turkish citizen, i’m sure if i do alot of research i can find a good clinic, but yeah hopefully my ortho believes my larps.

Yes in a case where you're having jaw surgery afterwards it's fine to do.

never read anything that associates the amount of movement with necessity for extraction. I think its just whether or not theres space to retrocline your incisors back or no. Basically whether your crowding is too bad or not.
