Med is just a shorthand of high protein, high MUFA, high vegetable and low carb.

Not rocket science why people stay slim eating med diets.
Though personally, I substitute high fish with poultry/meat and noticed no adverse effects. I still do eat fish about once or twice a week.
Fish is more natural for people living in med climates because they have such easy access to it.
Notice how in a typical western food pyramid, useless junk carbs like pretzels, pancakes, croissants, white bread and wheat cereals make up the entire largest portion of the graph.
The dairy and farmer lobby has great power over these graphs and is one of the culprits in keeping people fat.

Were you to eat like this, you'd be getting ~40g protein daily with 500+g of junk carbs and tons of hard fats.
