I don't want to preform a DIY medial canthus cut, but what about getting medial canthus. Obviously it's probably over priced, but I believe the medial canthus can really help the eye area. Is it a risky surgery? Is it worth it since it doesn't heal back immediately?

What about your lower eye support? If your lower eye support is poor, the medial canthus won't be visible enough, even if you have good medial canthus genetics.
