Hi, thank you for your recommendation. The money is like I said not the problem but I’d definitely prefer a more modular, lower-risk approach, I’d like to know your take on it though so I’ll share what I’ve planned as of now:

BSSO with ramus widening + sliding genioplasty to fix mandible projection, jaw angle, and asymmetry

Infraorbital + upper orbital fat grafting or implants

Medial + lateral canthoplasty to lengthen and shape eyes plus downward medial canthus resulting in a striking eye area

Supraorbital implants (if fat grafting doesn’t give enough depth or maybe a combination of both)

Possibly MARPE/MSE for mild arch widening

Would love your take on whether you feel a less aggressive approach could achieve 80–90% of the aesthetic correction, or if you believe LeFort III is truly essential based on your surgical experience.

Btw I do understand that LeFort III could address everything at once, but I’d like to confirm if my eye bulging is skeletal or soft-tissue related first (will get CT + ophthalmology clearance) and mainly make sure if I have less riskier options to take which can yield the same benefits as the lf3 without immediately jumping to lf3

So I’m planning a trip to LA to ascend in a few months and there I will have a consultation meeting with both my surgeons, one for orbital rim implants and canthoplexy and the other for genioplasty (sliding genio) and bsso mandible lengthening. I’m wondering if I should get fat grafting additional or as a general alternative opposed to the rim implants;
Here I’m planning to do mainly Supra implants but also ask the doc for his option on infras and other eye area enhancing procedures.

For the mandible length and chin projection problem I have I’m considering double jaw surgery instead of just sliding genio, and what do you think abt the bsso surgery for jaw lengthening? Can this fix asymmetry aswell?

As far as orthodontics go I’m planning on a very subtle expansion of the upper palate using a MSE expander for under eye support, smile mogging, oral volume, dental arch but mainly to even out the unevenness I have going on and after this i plan on getting invis align plus teeth whitening to finish the whole ascension off.

My starting point you can see as reference in the added pictures, I’ve taken multiple from various angles rate give opinions and most importantly give surgery suggestions or other advice to slay

sounds pretty good. make sure to get implants and then fat grafting. also, u dont wanna have an overly sloped brow ridge so i wouldnt recommend supras unless ur gonna do a very conserative forehead implant alongside it. not sure of it tho, js my personal opinion.

yes itll help a ton. lefort 3 is very risky, and is commonly used on syndromic or severely recessed people. u dont look too bug eyed, so i wouldnt recommend such an invasive procedure. take a look at this picture, look at all the nerves in the face
