Ting is saying he only expands until both up and lower bone are at the same width, you don't need to expand the jaw in this scenario. If you want more expansion on the maxilla then probably orthodontics will fix the lower teeth, with a bit of flaring and moving them around the alveolar bone. To match the forward growth he talks about rotating/displacing the jaw, also orthodontics.
