MSE is made as a functional appliance to fix breathing and narrow palate, the aesthetic benefits are a side effect and thus pretty subtle, (wider lips, better eye support, wider zygos) still there

Still the effort, money and pain is not worth in 90% of cases. You better invest 10k for some implants or other surgeries.
Small palate is only unattractive when you smile, and have visible dark mouth corners. The problem is also that getting mse with a normal bite is almost impossible without msdo. Mse is designed for people with a smaller palate than mandible (crossbite).

Depends on other things also. How much did you widen? Afaik most people go for very minor widening like 4mm. Also did he get the DOME procedure? Also did he use the facemask? Also we don't see the eyes and the smile either.

Agreed, for some ppl MSE is not worth it if all they want to change is aesthetics. If you have money and time it’s not a bad thing to try before surgery imo. I do think it it worth it for breathing problems and narrow palate (narrow palate can cause sleep apnea , tongue falls into airway during sleep if it’s not there 24/7) any other benefits are a nice addition. Sometimes even if your mandible and maxilla are the same size, Mse can be used to achieve 4-6 mm maxillary expansion because upper teeth are tipped outward and lower teeth are tipped inward. By reversing this you can get some expansion without mandible surgery.

Zero diff whatsoever, I think to get a good diff in your midface/zygos you have to do an implant as the MSE plus whatever midface osteotomy give minimal results
