What’s the diff between free test and regular test what crime did my test commit so it’s not free
Here’s a clear breakdown of the difference between total testosterone and free testosterone:














1. Total Testosterone








  • Definition: This is the sum of all testosterone in your blood.
  • Includes:
    • Bound testosterone (most of it):
      • ~60–70% is tightly bound to SHBG (sex hormone-binding globulin)
      • ~20–30% is weakly bound to albumin

    • Free testosterone (only 1–3%)







🧪 Total T = SHBG-bound + albumin-bound + free testosterone





  • Used for: General screening or diagnosis of testosterone deficiency (hypogonadism)
  • Limitation: Can be misleading if SHBG levels are abnormal (e.g. due to age, thyroid problems, obesity)
















2. Free Testosterone








  • Definition: This is the unbound testosterone circulating in your blood.
  • Biologically active form – can enter cells and activate testosterone receptors.

  • Used for: Evaluating symptoms when total T is borderline or when SHBG is abnormal.
  • More reflective of how much testosterone is available to tissues.
