Preface: Brief recovery guide & insights on side effects and what to do I wish I knew beforehand.
1.Introduction
Facial adipose distribution plays a critical role in aesthetic perception, and excessive buccal fat contributes to a less defined facial contour.
Fat dissolving injections, typically formulated with sodium deoxycholate (DC), target localised deposits of subcutaneous fat through lipolysis.

2.Context
I opted for Kabelline to target buccal fat & the perioral mound. It is a much milder lipolytic formula compared to Aqualyx, with weekly injections. The benefit of Kabelline is that it has a considerably lower DC concentration, reducing recovery time to 3-7 days and allowing you to go outside without looking like you’re severely deformed. Due to the lower concentration, more sessions are required.

3. Prevention & Side Effects

3.1 What to avoid?
1. Heat avoidance (72 hrs) → Heat encourages the solution to migrate and increases bruising risk
2. Alcohol consumption (24 hrs) → 45% higher bruising incidence
3. Side sleeping → Solution displacement resulting in asymmetric results
4. Over exercise → Accelerated formula metabolism → Swelling (walking and light jogging is fine)

3.2. Side effects I experienced:
- Haematoma (bruising)
- Lumpiness at injection site
- Numbness
- Mild facial paralysis

All of these were temporary, expected and nothing to be concerned about. One thing to note: upon injection, if the solution is placed too superficially or a large amount is injected, it will form lumps. This isn’t so concerning as long as the needle is within the subcutaneous fat and past the dermis (1.5-5 mm deep). This usually resolves within a week, if not a few days, followed by light massaging using your finger directly on the lumps with about the same force used to crush an eggshell. Numbness and difficulty moving your face is also normal; I had this after every session. If it persists after 24 hours, however, that indicates nerve damage.

4.Recovery guide:

The metabolic cycle is a 3 phase domino effect. First, the injected solution (usually deoxycholic acid derivatives) breaks down fat cell membranes within 72 hours. Then, your lymphatic system flushes out the debris over 2-4 weeks. Finally, collagen remodelling tightens the area, that’s why full results take 6-8 weeks. Swelling peaks at 48-72 hours because your body is mobilising broken down fat.

1-3 days. You should drink plenty of fluids (2-4 L) and gently massage the treated areas after your treatment. Simple analgesia (Panadol/ibuprofen) and ice application for 10 min every hour will make it more comfortable, but most patients don’t need it. Don’t use creams, makeup or sunscreen on the area.

3-7 days. Massage/lymphatic drainage is recommended to speed up recovery and decrease lumpiness. Continue with fluids and follow with light cardio to flush out debris and decrease swelling.

7-14 days – You will notice that swelling is improved and bruising should have subsided (if any).

Results:
Obliterated my perioral mounds (those fat pockets under your mouth) and reduced chunny cheeks, basically nonexistent now. Recovery took me 3 days to get back to baseline and 2 weeks to notice aesthetic improvements over 8 sessions totalling 8 weeks.
