My guess would be APT putting pressure on the veins, especially if you were very active growing up. All roads lead back to fascia. Less likely but possible it could be from riding bikes (pressure on scrotum), excessive mastrurbation (long drawn out sessions), or testicular trauma.

For me ig it went good rarely any pain and the varicole vein is not really noticeable even after hot shower
