It's genuinely over. I have a resting frown face at all times, my lips are genuinely so narrow, and I don't know if this is because the palate is narrow as well, but it messes with the harmony so much. Commissuroplasty is not an option for me because the results leave scarring, and it just doesn't look right. I have an appointment with a very experienced ortho surgeon who expands both the upper and lower, which would be doing an MSE, and also provides DAME lower expansion by splitting the mandible in the middle of the chin and slowly expanding it outward without rotating the condyle. And just looking visually if I open my mouth the pallet is very narrow and I've never really been able to chew food correctly since I've been a kid. neither is my airway good; hopefully this works out for me cause this is just fucked. I have to walk around with a half smile everywhere I go. Anyways, here are some photos of his before and afters

Could u talk and explain in details MSE and DAME?
I dont got an issue like u do but my pheno is mediterranean so dolichocephalic and narrow horizontally, long vertically
id like to get especially the maxilla little expanded so that zygos get more prominent and balance out a little the middle third (id like to avoid implants there) for the lower jaw never heard bout DAME i guess here in italy isnt even an option... Id like to learn more about this. I looked online about SArpe, marpe and MSE and everyone has fucking different expererience with it is very dangerous becous its easy to mess up maxilla symmetry. Furthermore, aside from the positive effect on the cheekbones, it can create problems in the orbital area. Some people have suffered from strabismus or other visual deviations that required other interventions. Another thing to consider (again, regarding the upper jaw) is the risk of widening the glabella, which in my case would not be beneficial. Finally, it should be noted that with orthodontics, once the palate has been widened, the teeth must be realigned. At that point, if the teeth were not sufficiently crowded, there is a risk that, by aligning them, they would become too far apart, which would significantly reduce aesthetics. Alas, in my case, I only have noticeable crowding in the lower jaw. I have a balanced amount of space above, but I would like to add a few mm of palate, you know... Oh, let's not forget that widening has been reported to carry a risk of asymmetry caused by an unbalanced division of the jaw joints. This too would be a problem. Finally, I recall that in some cases, orthodontists have been unable to perform effective skeletal augmentation. The result is that the teeth become misaligned from the verticality of the bone, so the jawbone hasn't actually moved, and instead the teeth have become externalized.
I believe this last thing happens especially in adults.

He is one of the only people in the states who provides MDSO-like expansions; he came up with a solution for the lower jaw because when you expand the upper, what do you do with the lower? It would creates a malocclusion and uneven bite issues and might have to do with teeth tipping, but this doctor made DAME, and remember DAME will not move the condyle, so that means there will really be no or barely any increase in bigonial width, for that you need implants but it will widen the chin and just make your lower jaw wider so it can match your MSE expansion; therefore, your bite will be good and your skull will become much wider. and you can really only expand up to 2.5mm
