My posture sucks

My posture is terrible, and because of a curvature in my lumbar and thoracic spine, I’ve lost at least 5 cm in height.
I also have one shoulder lower than the other, and overall my whole body is twisted to the side and my shoulder blades stick out.
How can I fix this, guys. Does anyone know any good stretching exercises I should try? I have a referral from my doctor for a spinal X-ray, and I might need further physical therapy or surgery who knows:hnghn:
 
I would work on doing an exercise that targets the erector spinae can improve your posture, personally I hold a plate with one hand and lean to corresponding side, which works for me. Beyond this I would get an X-ray and partake in the follow-ups with whatever the next steps might be.
 
Popracowałbym nad ćwiczeniem angażującym mięśnie prostowników grzbietu, które może poprawić postawę. Osobiście trzymam talerz jedną ręką i przechylam się na odpowiednią stronę, co mi pomaga.
