What do you think? I'm seeing an expensive well known surgeon, so I want the most aesthetic result I can get. The first pic is me now. Movements listed in top left of second pic.

No surgery for your nerd neck. You also need something for cheekbones, orbitals, philtrum, small mouth and probably for eyes. Will be a very expensive transformation. I am thinking about 100k+ in total. btw how is your hairline? You look like you could have a strong nw3. Am i right? Double jaw surgery will not bring you to a "attractive guy" level. Only average normie.
How tall and old are you? If you are manlet, i dont know if surgery will be worth anything for you.

My main concern is my jaw surgery at this moment, as it will set the base. That is what my question is... it's not about the rest of my face. The jaw surgery should impact my philtrum, since my maxilla is being advanced and downgrafted.

Yes my hairline is not good, and I already had one transplant in order to restore the hairline. I will be getting forehead lowering, as well as an additional transplant in the future. As well as other surgeries. But right now, I want my maxilla and mandible in the optimal positions.

I dont understand the surgeons plan, besides the genioplasty. What does he want to perform on you? Lf1 & bsso?

His neck has collapsed due at his recession.

His philtrum will improve will ccw, so he doesn't needed other surgery to fix it.

Yes, double jaw surgery will give you a strong base for other surgery to ascend, but only one condition: you should to do a HUGE MOVEMENT.

As I can see your surgeon is really conservative. especially with genio (2MM? Are you kidding me?).
In my opinion you should get at least 7-8mm of BSSO, 6 or 7mm of genio, more ccw.. of course I should see your CT Scan to tell you precisely.

He is a bit conservative with the genio. He mentioned 8mm of total advancement of both jaws, and seemed open to a couple more. I'm going to shoot for 10mm.
 
definitely go for as much advancement as you can get. Good luck with the surgery

10 mm is a useles movement for you.
As i said, you should get minimum 8mm of BSSO and 7mm of Genioplastic (with vertical projection) to get an aesthetic result.
If you get lower, you're coping.

Well 10 mm BSSO is more than 8mm, and the backloaded movement (BSSO) is more crucial than front loaded movement. Maybe I am misunderstanding you, but how is a 10mm movement useless if you are saying I need atleast 8? I do agree more than 2mm genioplasty would be beneficial.
You should lean down and soft max first tbh.
I'm not fat. My hyoid is low and my jaws are recessed and make my face look melted.

Drop below 15% bf move head and neck back/chintuck so you don't look all curved. Your spine should be straight.

Then obviously yes ALSO get this surgery, you don't have enough forward growth.
 
lots of downgrafting and CCW rotation, smells like a Gunsen special. How backlogged is he these days? Will he give me a ceph analysis via online consult?

LOL. I'm not sure. I highly doubt he will do an online consult but it is worth asking. Outside of getting as much advancement as I can, I'm just a bit concerned about my chin height, but my surgeon didn't see it being an issue.
