There’s basically only two legit plausible theories

1) it’s an optical illusion due to decreased fat pad above the penis

2) the skinnier guy has less plaque in his blood vessels thereby allowing more blood into the penis and harder erections (bigger)

I guess you could say testosterone is higher in lower body fat too but test wouldn’t actually make a dick bigger it would just make it harder (example 2)
