If in a setting where people have seen you multiple times (school, work, etc) and they have to ask for your name, it's over. Think about it, if you were appealing they'd find your name out some way (probably by asking someone). If you're an NPC, they don't care what your name is. If people in places you frequently don't know your name, it's over.

Same, I have a fairly long italian name and teachers can't ever pronounce it

Yeah another part of the name pill is of you have a really low liked named, such as Gilbert, where it's bad enough that people could reasonable say the phrase, "But your name is ____" as a comeback, it is brutally over.
