There are different stages and some are self aware.

Really bad narcissists probably would not be self aware enough to change their answers. There are people with bad enough narcissism that even if they hit your car trying to park they would yell at you for parking too close to the line and demand you pay the damages on their car. These are the types of people who if you asked them who the smartest person is they would unironically reply themselves even if they are 80 IQ they would still believe it genuinely

Chris Chan is an extreme narcissist for example and believed he created the entire world and could bring to life animated characters. People generally think about smart cunning narcissists but a lot of them are people who are just extremely delusional. The dangerous ones are the people who only care about themselves but also are self aware on some level they can hide it quite well these are your politicians, ceos, lawyers, and billionaires.

Narcissism is not a fun trait to have if you are stupid or incredible low self awareness and egoistical. A lot of autistic people over the years I have noticed are like this unbearable to be around them. I have a lot of experience cause I had to take care of an autistic cousin she was 25 very low functioning. She was not very smart but thought she was a genius. She didn't have any ability at all to think beyond what she wanted. She would lie constantly even though it was obvious like she would eat cake and have crumbs on her and say the dog ate the cake. And she would always cry about her autism making people dislike her when she was terrible to be around.

I hated having to watch her she was literally incapable of viewing herself as wrong even when scolded. She spilled coffee on me and got mad and cried to my aunt that I ruined her coffee drink when she spilled it on me accidentally. There was never a point where she could even comprehend anything but herself that is narcissism in its purest form which is not useful but people who learn to conceal it and are semi self aware often do better because of the trait. If she wasn't family I would have hit her or beat her up she pulled my hair a lot of times when she was mad. Aspies are cool but low functioning autistic people like Chris Chan are some of the worst people imaginable. Nothing redeemable at all about them

But a narcissist who makes it far in life is aware he is not normal and is playing a game of among us. These people are always putting on various masks to try to fit in and are incredible good at it. So they are either low in narcissism and will just believe they are always the best person in the room whether this be the best morally I'm such a good guy or the smartest ect. Or they are aware of this condition and just try to hide it
