need help rethinking surgical plan

So, I had a consultation with Dr. Ramieri today. He said he wouldn't do bimax or that in order to do it, I'd have to have premolars removed and braces for a year, but he sounded like he did not want to do that or think it was a serious possibility. Now before some of you get all "I knew bimax was a meme!" I am birdcel, not BDD:

That's what I looked like at 18, pre chin implant. This is what my skull looks like:

It actually doesn't looks as recessed as I thought it would based on my appearance. The thing is, I'm fucking hideous and I need surgery. I need rhinoplasty revision. I need every implant. I need braces. Although he said I had a decent chin implant result, Ramieri said he could replace my chin implant with custom genio for $10k. This would allow me quick access to Dr. Marianetti for my rhinoplasty revision, which I desperately need. My nose is hideous and I'm too embarassed to even post pictures of it.

Should I get a 2nd opinion? $10k seems a lot when I could probably just get a better implant when I get all my other implants. Or should I get the rhinoplasty revision + genio, and then get custom implants from RealSurgeryMax + Pagnoni in a 2nd surgery. Although RealSurgeryMax doesn't seem to be very responsive. Any feedback or advice?

A chin augmentation with a customized approach is probably best in your situation, even though it's the most expensive.

Yeah. But if I get custom jaw, cheek, orbital, paranasal, then custom chin implant isn't going to add another $10k onto the price like the custom genio would.

Will probably be 4000 for a good one. Mb, ofc it won't come close to the 10k.
