75% of patients WILL need a facelift after infrazygo implants.

You MUST plan on it.

There is NO alternative.

You mean that it shines trough the skin, right?

A lucky minority will experience exclusively that issue. They can get away with doing exclusively fat grafts to solve it.

All patients should be sure to have enough money to do a midface lift after their implant!

Do
NOT get an implant without being prepared to need a midface lift after! Under NO circumstances! You have been warned!

I am not getting any implants in that region as it has the thinnest skin compared to the infraorbital region where muscles and fat are more present.

But yes, you should always have some money backing you up if anything goes wrong.

I agree. I am happy to share some good and economical (under 20k) midface lift surgeons for all those who are planning to get infrazygo implants!

Did you get one (or know someone that got one) and were the results good?

Yes I've seen a few!

The possibilities of ascension are crazy for those with a really big budget.

The reason is that many people with recessed bone structures, even who are young, have naturally low set soft tissues as well.

You put an implant in, the periosteum and fat pads do not sit on the new bone structure in a natural way. You end up with the bone structure of a handsome guy, but not the soft tissue drape of a handsome guy.

They must be manually lifted up.

The majority of implant surgeons today do not manually lift up the tissues as part of the procedure. This was de rigueur with Yaremchuk before his retirement.

It is especially necessary with PEEK implants which are more prone to reflecting light and exposing themselves than implants made of other materials if the soft tissues are not appropriately draped over them.

Anything short of a full-fledged subperiosteal, deep plane, or SMAS midface lift performed by a dedicated soft tissue specialist is UTTERLY insufficient
.

Under no circumstances should anyone entertain discussion of infrazygo implants without also discussing midface lifts. The two procedures MUST be viewed as IRREVOCABLY UNITED. Having an infrazygo implant without having a midface lift is like making a cake without flour. A veritable surgical umbilical cord attaches the infrazygo implant to the midface lift.

Great explanation, makes perfect sense.

should u be prepared for fat grafts and midface lift?

what about jaw implants? Are they fine if done alone?

No. Dehiscence of the masseter muscle is nearly always guaranteed. Its appearance can be reduced with fat grafts and/or Botox as well as keeping the implant as SMALL and CONSERVATIVE as humanly possible. However, this problem typically does not affect your looks as much as this issue with infrazygo implants does.

Please also consult the section on cheek implants in Michael Yaremchuk's Atlas of Facial Implants to learn more about the utter necessity of performing subperiosteal midface lift alongside all cheek implants!

I mean ofc it can be good, but its not necessary in many cases, it depends on how good elasticity you have before like you said. Face lift is probably needed if you are 30+ but not younger

No, it is still absolutely needed in young patients, if marginally less so. It is not sheerly a matter of skin laxity or elasticity, it is a matter of the way that soft tissue drapes on your skin due to the placement of ligaments and natural shape of your soft tissue envelope. And the place and shape of those things is reliant on your pre-existing bone structure. Even before aging sets in, soft tissues naturally sit more low on recessed faces than they do on projected ones. Cheek implant without a midface lift--to drape the tissues nicely over the new contour even if they have not sagged due to aging--is like eating raw chicken. Eating raw chicken without cooking it!

Dr. Yaremchuk did subperiosteal midface lift in ALL cheek implant patients, regardless of age. Dr. Rad says he does midface lift in 75% of cheek implant patients. I hereby propose to abolish the terms "cheek implant" and "midface lift" and combine them into a single word "cheekimplantmidfacelift" (or in German: Wangenimplantatmittelgesichtslifting) so that we can move out of the Dark Ages when people were able to even conceive of the idea of these procedures not having to always, without exception, go hand in hand.

It is an incredible procedure! One day, if I get a cheek implant myself, I will get the midface lift too.

Yaremchuk and Oscar Ramirez almost always did subperiosteal midface lifts with midface implants. Otherwise the subperiosteal dissection will lead to some degree of tissue descent. Unfortunately not many surgeons in the US who are good at midface lifts anymore, and even fewer who also do custom implants.

Lmao I’m the perfect failed experiment, and the Doctor you mentioned was the scientist. Almost died and I was one of his last patients. Look at my posts, my cheeks looks extremely fat and loose, even Pagnoni told me the soft tissues of my face are hanging like a curtain. Replacing my implants with him then getting a deep plane lift in May. Rip my social life for the next year and a half

Are you sure Yaremchuck always did the mid face lifts? I was essentially one of his last last patients, spent a lot of time with him since I got an infection twice and had to remove one implant, then replace it six months later. Total of three surgeries and we ended up talking a lot, he’d even invite me to his clinic on days he was closed, he ended up getting a soft spot for me due to my situation.

That aside, I have realllllly bad midface ptosis, infection ate the ligaments of the corners of my eyes and I have a deep scar on the corner of my right eye, the size of a dime. I spoke to the top referred surgeons/designers in this forum (not gonna say names but it’s probably who you think it is and they all told me I DESPERATELY need a midface lift and to replace my infra zygo implants.

Looking back at it at this point I don’t even know if Yaremchuck was at fault, I know he tried his best with me but I’m not going to blame him for all of my problems even though I’ve essentially lost 3 years of my life and I’m going to lose and year and a half recovering from replacing cheekbone implants then getting midface lift 8 months after

replacing it each time increase even more the laxity btw

It probably isnt the case, and not even the guy he send did it. He did a under eye bleph, which ofc lift the skin slightly. But yea giant has started doing it now apparently

how long could you/should you wait before getting facelift?

The problem I have with this is zygo/cheekbone implants already are said to be quite high. THEN having to pay for a whole face-lift after is just insane, then factoring it that isn't a high probability of some type of scarring of the face lift? + the unnatural look it gives? So it's wild to me.

The surgeon told me it’s best to wait 6-8 months after you get your implants. He said they could do a midface lift and the implants at the same time but that it’s essentially guessing, so he doesn’t do it. After the soft tissues settle a bit from the implants, the surgeon will have a better view and idea of how to reposition your soft tissues to get the best result.

It sucks in a time perspective, 6-8 months after zygo implants then 8-12 months for swelling to go down from midface lift. You see your final results a year and a half after everything lol. I would of never gotten my implants if I would of known this but I’m already trapped in this situation due to my specific circumstance.

Yeah I’m essentially forced to replace them, since the implants are so large that they’re dragging my eyes down, especially since my ligaments were severely damaged. Only way to fix my eyes is replacing with a much smaller custom implant that’s designed around the midface lift and canthoplexy in mind

Well what else would one do for cheekbones if it isn't that.

It sucks either way considering their is nothing you can do to counter having zygo implants if you have a death tier FWHR. So there's that.

I agree, I wouldn’t have minded waiting one year and a half if I didn’t already didn’t wait that time in the whole process of removing the infected implant - waiting 8 months to replace it - then waiting 8 months for the final result. This is going to be a three year process in total but it’s taught me patience and to be grateful for what I have
