Post-bimax antegonial notch

Some surgeons put graft material on the antegonial notch and some don't. This notch bothers patients to the point some of them get jaw implants after the bimax for the sole purpose of eliminating it.

Why do some top surgeons purposely NOT fix the notch with graft material? Is there any safety or aesthetic concern? I have consulted with some surgeons who fix the notch and others who don't and I still haven't been able to clarify the issue.
 
Its called a step off and it depends on the movements created. Everyone doing bimax has a different beginning point, different movements designed by the surgeon, and different result obviously.

You can do implants yes or a design to work around it.

It's not really an issue. Usually the way surgeries work is they fix one issue and cause another minor issue then you get another surgery to fix the minor issue.
