Recently, while browsing through PubMed, I came across an interesting study and wanted to share a theory I had about it with you guys!
The Case Report :
A 22 year old woman with a 4 cm odontogenic keratocyst in her left mandible underwent cyst enucleation and AutoBT. The surgeons combined these two procedures with rhBMP‑2, a protein that helps the body to grow bone.
Miraculously after a period of about three years the bone defect didn't just heal it also remodeled into (cortico‑cancellous) bone.
The newly formed bone looked exactly like the surrounding bones and there were no negative outcomes, with no sign of recurrence.
My Theory :
As we read in the case of the 22‑year‑old woman, who had a cyst in her mandible, the human body is able to replace a bone defect with almost identical cortico‑cancellous bone after filling it with AutoBT and rhBMP-2.
If this bone remodeling method works for bone defects, then intentionally creating a controlled bone defect and filling it with the same osteioinductive graft, which could heal into new bone and potentially, if my theory is correct, leave you with more bone mass than you started with.

Osteoconduction(the structure):
AutoBT is made from extracted theeth, often from the patient itself. Theeth are mostly made out of collagen and hydroxyapatite, wich is nearly identical to human bones except for some minor differences like the type of collagen. When the AutoBT is placed in a defect it acts as a support structure. Osteoblasts, cells that build bone, move onto the support structure and start forming new bone matrix. The material doesn't attract actual bone forming cells, it rather gives them a structure where they can build and grow on.
Osteoinduction(the signal):
RhBMP-2 is a growth factor wich brings in mesenchymal stem cells from the bone marrow and soft tissues. The body then turns them into bone forming cells (osteoblasts). It does this by binding to cell surface receptors and activating intracellular signaling cascades that turns on bone building genes.
increased bone regeneration:
Normally a bone defect is by bone growing from the edges. But when a support structure like AutoBT is present and a osteoinductive factor is added, the healing of the defect can exceed what is needed to fill in the cyst. This happens because the body has more growth factors than needed and lasts long enough to activate more osteoblasts than necessary.
Conclusion :
Pros :
This study is a single case report. The procedure was performed on one woman, and the report was only published 1 month ago, so it's unclear if this is safe or not. If this theory actually works, then this procedure could be a great alternative to fillers because the results would be permanent wouldn't disappear.
Cons :
The negative though is that it takes a long time until it's fully remodeled and healed.
