Prominent ANS vs Maxilla projection : Class 2 Deepbite DJS Prep in France

Wisdom teeth removed + soon decomp ortho with French ortho who works with my surgeon

- Im really conserned about my Nasale spine (ANS) who is really projected and make my mandible appear even more recessed
- Was thinking ~10mm mandible + ~4mm maxilla + little cw for reducing deep nasolabial fold as Im class 2 and deepbite but not too much since I have pretty narrow jaw from front, and I also think the maxilla is already pretty good

The real question is I've never seen anyone with the same nasale spine as me.

also wondering how to manage the projetion of my nose tip and my upper lip with this nasale spine

What do you think should I go with the planning?

Subnasal spine leforte cut seems obv in your case

not sure it's necessary to touch the upper jaw at all. You need decomp+bsso.

ye you're right, but Im not shure if I can have a decomp thats more than 8 mm, so the bsso will be limited

Shaving is crazy
very common tho, even heard of getting subspinal lefort + shaving it down for aesthetics cuz of too much nasal spine projection and more controlled change, which OP would benefit from

Cuz he says it’s to add nasal volume

crazy, slight adjustment would be fine to hear, but fully shaving it down aint it

You think shaving it like 2-4mm would be good depending on the maxilla movement ?

Yea I was speaking w some hyperbole idk if he completely n fully shaves it all down but considering how much he advances ppl, overly so, I can’t imagine he proceeds w too much restraint on the shaving

did you consult a surgeon? whatd he say
I put the braces on in 3weeks. I already consulted him, he said we will see after decomp to know how much movement on the maxilla since it’s depend on the gap between teeth. We haven’t talked about the nasale spine

yeah i was fucked up a little before. You niggas like to tilt your head to the moon so its hard to assess precisely. Your overbite is actually minimal so you will need a bimax for a noticeable result

Your maxilla has a really good shape. Why are you cutting it? A prominent ANS is an attractive feature. I think you only need lower jaw surgery to get that thing a little bigger; your jutting looks like a morph. Save yourself the expense, and think of BSSO and genioplasty or implants for the jaw angles and the chin.

The mass of your jaw is the difference between you looking like bratty punk and male model.

I agree with you, but maybe the decomp won't be enough to create a large enough shift to advance the mandible by 1cm like when I’m jutting. I would prefer not to touch the maxilla, but I'm not sure if that's possible if I want a 10mm advancement of the mandible.

Your chin is like a letterbox or tip of a pencil at it's most anterior point. Some volume with bone cements just above it or however surgeons deem best is how they can solve it. There needs to be volume in that area. I've attached an edited picture of your CT in frontal profile to show you what needs to be added. Genioplasty won't help when your chin has a chisel tip. It looks like you could ice a cake with your chin, or do calligraphy, or keep a door open with it. The mentolabial fold that you dislike is caused by a concavity inside the current bounds of your jaw that experienced surgeons can fix easily, and can do it while in conjunction with a genioplasty that increases the tallness of your chin.

Ah yeah, I had never noticed this problem with my chin. So, like you said, I need volume to be added just above the bottom of the chin. Thanks for the image. For the mandible advancement, no, I'm not sure it needs to be 10mm, that's just an estimate. I see you really don't like my sharp chin :lul:. And so with the volume added, do I steel need downward rotation too on the chin ? Thanks for the advice :love:
