Question about canthoplasty

I wanna get a canthoplasty sometime in the future. The problem I see is that canthal tilt is determined by bone structure but in the surgery they don't change the bone. I'm wondering if that's the only type of canthoplasty or if there is one that makes the bone look like somebody's with a natural positive canthal tilt. And how risky is it if it exists?
 
the lateral canthal ligament determines your tilt not bone
