if i get a custom one, so it could look similar to what cox or maher have, is that possible? or there are limitations like downward growth or something else

you can definitely, if the surgeon gets the lateralisation component correct (basically, how much of the side part of the face is visible from the front). Often I see this is what is lacking in jaw angle implants, they either go overboard or ignore this part)

still, one major issue with implants is that in naturally well-grown faces, the bones and ratios change in tandem

implants to correct major downward growth can indeed look uncanny.
