I'm getting bimax + genioplasty in 3 days at age 17. I'm scared of underadvancement, i get it performed by Dr Kater, he's very experienced but also rather conservative. I'm having a last consultationtalk with him 1 day before surgery, any ideas on how i can convince/show him, that i want more advancement and i'm not scared of big changes. Are pictures of good results the way or is that just gonna diminish my credibility?

bigger movements arent possible since u didnt decompensate with braces + u will probably need rhino after since not only your pre op nose is fucked but your alar base will widen a bit

The genioplasty and ccw rotation will have to carry

ccw rotation will not have a big impact since ur occlusion plane aint steep. more or less the genio has to do the difference in your case

do you think it'll still be a big ascend? and do you think decomp would have really made a big diff?

both aesthetic and airway wise yes. u usually want to do 70/30-80/20% jaw/chin movement. it will obviously improve but your surgeon has to do a big genio to decompensate for the chin projection

so you'd say decomp would have made a big difference... i already had braces for 3 years and i just can't live with this recessed jaw anymore knowing that there are ways to change it... do you think the bimax will be underadvanced or do you think i shouldn't expect that much?

it will make a difference, but decompensation is something very important to see the best results. every good surgeon knows how important it is including pre ortho treatment for bimax. people do this type of surgery at average at 20-25 yo. u could do genio only now and decompensate after for bimax, but since your surgery is now just do it

i already had 3 years of orthodontics, do you really think the camouflage is that bad? i had a herbst hinge that advanced my mandbile forward so my occlusion being class 1 is not just carmouflage

both countries (UK & Germany) are run by ... that replace white people with muslims so both countries aren't important

u probably need lower premolars extraction. i got it too, im currently in decompensation to get a class2 bite for bigger mandible movement in relation to maxilla movements. if seen interviews of top surgeons being asked why getting big chin advancments but not jaws is bad. it aint a chin problem if u need 8-10mm of a genio, its the jaws. surgeons like gunson,bobek, raffaini,.... are doing BIG jaw advancements with small chin tweaks.
 
I don’t see why he would need dental decompensation, his bite is normal class 1 with no malloclusion.

I don’t see any reason to extract premolars and drag the lower incisors back to do a larger BSSO rather than a larger Genio. Especially as it will mess up his bite in some ways.

I don’t see any reason for CCW as the occlusal plane is already correct.

In fact I barely see any need for bimax. Movements will be equal lower/upper and tiny.

Genio will do almost everything.

you're right... decomp would make it much better but the surgery is in 3 days... i don't know what to do, you only get 1 shot with bimax

His back molar occlusion is correct though, BSSO will fuck it up.

My maxilla isn't even recessed, so if you see it from that perspective the Le Fort 1 is doing minimal changes (just a slight ccw rotation) and still has a very big risk

did you have lower and upper teeth removed with the braces before?
No just a herbst hinge, that advanced my mandible forward for 2-3mm and normal braces
 
have seen enough cases where they did it, doesnt matter at the end.
i still don't know if bimax is the right choice and how much i can/should expect

do it if surgery is next week. if your surgeon does slight ccw and the 8mm genio u will probably see a difference of almost 1cm at the lowerjaw/chin in relation to the upper jaw.

do your parents have the some hooked nose?
 
1cm difference is not that easy in my case and it would still leave me recessed, so the smarter way would be to cancel
 
then u probably have some maxilla deficiencies. your nose is the way it is cuz of the upper jaw not giving the support your nose needs.

Jews are an ethnic group, and Islam is a religion. Both groups are Caucasian, their skulls are indistinguishable from Europeans. Also why would Jews want to replace whites with Muslims, obv Muslims would be harder to control. The replacement theory makes absolutely no sense.

bro i don't even mean jews... i don't care who replaces whites with muslims, even if it's not out of pure evil but just out of stupidity... the replacement is just pure logic, muslims get 5 children per family whites 1, that means muslims will outnumber us eventually and vote their own sharia law partys leading to the extermination of white people.
