Ramieri Surgery Coming Up, Giant Implants is Designer

I had an emergency Zoom meeting with Dr. Ramieri this morning to discuss everything.


The original 5:00 PM Italy deadline has passed, but Dr. Ramieri told me all hope isn’t lost. If Liam can get the finalized files sent over as soon as possible, ideally by Friday but even sometime this weekend, there’s still a small chance we can keep my July 10 surgery.


Nothing is guaranteed, but the situation isn’t completely over yet. Dr. Ramieri said he’s still willing to try if the files arrive quickly enough.


At this point I’m just waiting and hoping Liam comes through. I’ll update this thread as soon as I hear anything. Hopefully this isn’t over yet.

Basically to Giant maxx you have to be ready to hop on an international flight to go get surgery at any time…

Small update: I finally received the implant design from Liam, so that part is done.


Unfortunately, the original 5:00 PM Italy deadline has already passed. At this point, everything depends on whether Dr. Ramieri can get the manufacturer to approve and produce the implants quickly enough.


Tomorrow morning (Italy time) he’ll be speaking with them to see if there’s still a way to keep my July 10 surgery on schedule. If not, I’ll most likely have to postpone until December.


I’ll update the thread as soon as I hear anything.

I had to drop the rhino and will do it this winter alongside my ht. Surgery, anesthesia, hospital fee, Giant’s design, food, hotel, flight ~$36,000

Post-Op Day 5 Update


Had my first follow-up with Ramieri today.


Overall, everything seems to be healing well. The swelling has already come down quite a bit compared to the first few days after surgery, but it’s still far too early to make any real assessment of the final result. Right now my priority is healing properly, staying on top of my medications, and minimizing any chance of masseter-related issues. The real evaluation won’t come until much more of the swelling has resolved.


One interesting thing we discussed was the genioplasty. The original plan remained a 6 mm anterior advancement with a 3.5 mm downgraft. During surgery, Ramieri did evaluate whether an 8 mm advancement would work. After seeing everything intraoperatively, he decided to stay with the original plan because he felt pushing it further wasn’t the best aesthetic choice. I still want to ask him whether that decision was purely aesthetic or whether there were any technical limitations as well, but that’s my understanding of it for now.


One thing that definitely gave me confidence was hearing Ramieri speak very highly of Liam’s @RealSurgerymax design work immediately after surgery, especially regarding the custom mid-face implants. Hearing your surgeon compliment the implant design after everything was placed was reassuring.


On a completely different note, my body composition has gone to absolute shit 😂. I’ve been off retatrutide for almost a month, off testosterone for over two weeks, no GH, no gym for over a month, living on a soft-food diet, and basically not training at all. I expected it going into this trip, so I’m not stressing over it. Once I’m back home, everything goes back to normal and I’ll get my body composition back.


I’ve also been documenting the entire process on my Project Ascend TikTok page: https://www.tiktok.com/@facelessglow23 from the planning, financial side of things, delays, surgery, and now the recovery. I’m keeping everything anonymous for now. Whether I eventually decide to make it public, I honestly don’t know yet. There are reasons I’d like to stay anonymous, but we’ll see where things end up.


For now, it’s just a waiting game. Recovery comes first. I’ll save my real thoughts on the outcome until the swelling has gone down over the next few months. Right now, I’m just trusting the process.

How long has this whole process taken from first initial consult to actually the 10th of July ascension day? And how did you initially get Liam involved, through ram or by yourself? He’s obv a bit difficult to reach

First consult with Ramieri was around January/February this year. I had my CT done around October of last year. I could have gotten the surgery sooner but much easier for me during the summer cause of my work schedule and i didn’t have the funds anyway. I didn’t even know who Giant was at the time. When I came across @Linoob ascension on tik tok, I found Giant. He was looped in mid-March via an IG dm. When I found out he works with Ramieri, I brought him on board.

Aside from a few last-minute logistics before surgery, I have nothing but good things to say about @RealSurgerymax It’s too soon to speak on the results atm since I’m still quite swollen one week post-op but from Ramieri’s praise on the design immediately after surgery and how things are healing, I seem to be headed in the right direction.

Why did u decide against lefort in the end?
Ramieri said a lefort 1 alone would lead to malocclusion since I would then have to advance the lower jaw as well. And aesthetic ROI for a DJS for me wasn’t there based off the downtime and invasiveness. For the aesthetic results I was seeking, according to Ramieri and another surgeon I had briefly consulted with (Burak Ercin), implants would suffice.

Do u have a class 3 malloclusion but camouflaged?

Post-Op Day 14 Update


Had my third and final follow-up with Ramieri before flying back home.


Overall, everything continues to heal well. According to him, approximately 60–70% of the swelling has gone down, with most of the remaining 30–40% concentrated in the mid-face. It’s obviously still far too early to judge the final result, but recovery has been progressing exactly as expected.


No major complications so far, and each follow-up has been reassuring. At this point, it’s just a matter of being patient and letting the remaining swelling continue to resolve over the next several months.


The next priority is continuing to improve my skin. The acne scars have already improved quite a bit, but texture is still my biggest concern. I’ll continue with Fractional CO₂, erbium laser, and subcision treatments with my dermatologist in North Carolina over the coming year.


The next major milestone will be returning to Rome in December for rhinoplasty. Around that same time, I’ll also be traveling to Turkey for my hair transplant. Those will be the last major structural procedures before the focus shifts more toward refinement.


After that, the plan is under-eye fat grafting, possible canthopexy depending on how everything settles, eye color change (cornea depigmentation), veneers, subtle lip enhancement, eyebrow tinting, and continuing to improve my skin. I expect the skin work to continue well into 2027, since improving texture is a gradual process rather than something that’s fixed in one or two sessions.


The final major decision after that will be height. I’m still deciding between trying the Alex Kikel protocol first or eventually moving forward with leg-lengthening surgery.


I’ve also continued documenting everything on my Project Ascend TikTok page facelessglow23, from the planning, financial side of things, delays, surgeries, and now the recovery, along with the roadmap for everything that’s still ahead. I’m keeping everything anonymous for now, but it’s been a fun way to document the entire journey as it happens.


Looking back, it’s kind of surreal how much planning went into getting to this point. While there are still several major steps left, it feels good knowing the first phase has gone smoothly. Hopefully the next update will come with even more swelling gone.


For now, it’s just a waiting game. I’ll continue updating this thread as things progress. I still don’t plan on posting final before-and-afters anytime soon since I want to wait until I’m much closer to the long-term result.

1 Month Post-Op Update


Today marks one month since surgery. I had all three procedures done at the same time: custom infraorbital/malar implants, custom jaw angle implants, and a sliding genioplasty with 6 mm advancement and a 3.5 mm downgraft.


I sent Ramieri some current photos a few days ago and he said everything looks like it’s healing fine.


Aesthetically, I’m still having a hard time assessing things because my face looks pretty full overall. The biggest thing I notice is the midface. The area underneath the infraorbital/malar region currently projects outward rather than tapering inward beneath the cheekbones, which gives the midface a more rounded and convex appearance.


There’s also still fullness through the lower cheeks and lower third around the jaw. Overall, there isn’t much narrowing from the midface into the lower third right now, so my face looks considerably broader and fuller than I’m used to.


I’m not really trying to make a judgment on the result yet. Considering how invasive it was to have all three procedures done simultaneously, I’m mainly curious about other people’s recovery experiences.


For anyone who has had a similar amount of facial surgery done at once, how did you look at one month compared to months two and three? When did the swelling stop significantly affecting your facial proportions, and at what point did you feel like you could actually start assessing the result?


My current plan is to hold off on making any serious judgment until around the three-month mark, but I’m curious whether people who have actually gone through this think even that is too early.

An alternative to Giant is going to South American surgeons and consulting a professional designer whose network consists of those surgeons (Colombia, Mexico, Brasil). People can try www.facialnexus.com if they want the south american route. Consultation with designer is free.

Pros: better service, better care and aftercare, better guidance, lower price

Cons: People are afraid of traveling to south american countries. It's all propaganda imo. As long as you're not traveling with a suitcase full of cocain to meet the local drug lord, you're safe.

I never asked so not sure why you’re posting this here. And I can’t relate to the brokies here who can’t get access to Giant @RealSurgerymax or reputable surgeons lol. When your surgeon who’s been practicing for almost two decades tells you that someone else is a better designer than his own, and follows up with high praise immediately after surgery, it tells you a lot about ones ability to design implants.

People are flooding the same surgeons with requests, thinking that this or this specific surgeon can do a better job. That's just not true.

First of all, when surgeons get more and more patients, they will cut on time and care, and eventually they'll get so many clients, they'll be obliged to ghost clients when something goes wrong. There's only 24 hours in a day. The service, care and aftercare get less and less, at the same time, prices go up. This is what happens now, and this is not what you want. More botched cases, less care, less aftercare, much higher prices.

Secondly, the surgeon is overall, just an executor. Design and planning are the most important. Giant is unavailable for most clients, and he has been made a myth. You know when somebody gets a godly status, you need to substract from the mythical status to get the actual real life value and true results. When a person reaches that status, people begin to highly overestimate everything. It happened to Eppley too in the beginning days of PSL.

It happens in every domain. Trump called himself the best dealmaker in the world. You look at the MOU with Iran: this is the wost deal EVER.

I simply offered an alternative.
