What surgery u recommend, or implants etc etc I get it alot like you said but aside from cantho and fat graft I've gotten no other insights

I've also been told my lower 2 thirds are more brutal than my eyes hence I genuinely don't know what to go for at this point

tbh maybe you should get a laser facial hair removal those little hairs look unkempt and the shaving might play a part in your skin problems but def not the root cause but not sure if its really worth it for you also take into consideration if you might wanna have a beard in the future
