Bad skin with scars and very red, unappealing tone. Nose too wide for mouth, bad upper lip. Long philtrum. His cheekbones are really uneven and his orbitals are bad and rounded

For skin just tretinoin and microneedling would get you places.

About the Philtrun, unfortunately there isn’t really any direct surgical solution, but you could look into lip lifts which will also enhance your bad upper lip, but that surgery is volatile and can be botched so weigh your risks.

As for your mouth and nose ratio, I’d personally look for debloating the nose or look for mouth extenders, there isn’t really a good solution to this problem, and alar base reduction rhinos aren’t really good

As for the asymmetry, just lean down to 11% and it’s supposed to get better and also get rid of the bloated look
